Question:

1. Name?
   “Slide rule data slips” seems to be common for Koch, Huxhold & Hanneman(KHH), and a few Frederick Post rules Andries de Man
   The “Slide Rule Data Slips” title of the paper table on the back is perhaps a clue being reminiscent of an early (circa 1917) Nestler Mannheim (no model number) but made by D&P. David Rance
   Note that for this slide rule “Slide Rule Data Slips” is not followed by “compiled by C. N. Pickworth, Wh.Sc.” Andries de Man
